The following tools were developed in collaboration with UNICEF to support the planning, carrying out and analysis of LQAS surveys. They focus on maternal and child health and include:
- Sampling frame calculator, detailed implementation plan and budget template to help plan your survey.
- Indicator lists and seven modular generic questionnaires for maternal and child health and community health workers that you can adapt for your survey
- Hand tabulation sheets for each of the questionnaires and a standard reporting format to help you analyse your survey
The tools are open for all to use or freely adapt. The sampling frame calculator was adapted from an original model by John Marsden and Tom Davis at Food for the Hungry. French versions are available here

LQAS Budget Template
This excel sheet can be used to calculate the cost of carrying out an LQAS survey. Simply fill in the details of the survey on the “Information” sheet, and the file will automatically calculate the cost of the survey and break it down by training costs, data collection, hand tabulation, health facility assessment (if required), consultancy, and other expenses. The final page then gives a summary of all costs.
LQAS Sampling Frame Calculator
Enter the population data for the survey area into this sheet, and it will use Population Proportional to Size (PPS) to identify the interview locations for your survey.
LQAS Generic Household Survey Tools
This file includes an indicator list and six questionnaires to collect the data. The questionnaires are modular so they can be easily adapted to project requirements. Also included are tabulation tables to process the data collected in the questionnaires; one for the Supervision Area level analysis, and a summary sheet for analysis at the district level.

This is a template to produce a final report with the data you have collected. It gives a format for displaying the data, explaining the methodology and tips on analysing the results.
LQAS Videos
Researchers from LSTM have produced a set of videos explaining the benefits of using LQAS surveys in planning, managing and improving health programmes.
The videos have been produced professionally in hi definition. They can be played chapter by chapter or in one sitting and includes an introduction to LQAS as well as practical chapters on how to identify supervision areas, how to select a household to interview and what are the best interview techniques. Created to act as a companion to LQAS training, the videos offer a step by step guide to the system including a chapter reviewing what has been learned.
