This study aims to investigate the effect of smoking and Chronic Obstructive Pulmonary Disease (COPD) on pneumococcal colonisation. We plan to recruit 35 healthy smoking adults aged 18-50 years old and 35 adults 50-84 years old with COPD that are generally fit and well.
The participants all receive a nasal inoculation with pneumococcus and attend clinic appointments for up to 6 weeks. At the end of the study, participants that have become colonised with the bacteria will be given antibiotics. Participants will be paid for their time and inconvenience.

To test if genetically modified strains of pneumococcus can successfully colonise in the nose of healthy volunteers. To investigate if the genetically modified strains can provide protection against future colonisation in the nose after 6 months.
This Single Blind Randomised Controlled Trial SBRCT will recruit 150 non-smoking healthy participants aged 18-50 years old. Participants will be randomly allocated to receive a nasal inoculation of pneumococcus/ genetically modified pneumococcus or placebo (saline). The genetically modified pneumococcus has been modified to remove the factors that would be linked with disease. The participants will attend clinic appointments for 6 weeks and then be given antibiotics to clear colonisation. All participants will return for Stage 2 after 6 months and receive a nasal inoculation of pneumococcus to test if the previous inoculation has provided protection against further colonisation. Participants will be paid for their time and inconvenience.
